What is a Home Medication Review (HMR)?

A HMR is a consumer-focused and collaborative health care service provided in the community setting, to optimise quality use of medicines and consumer understanding. 
It involves the consumer, their general practitioner, pharmacist, and other relevant members of the health care team.

Steps involved in a HMR (MBS Item 900)

  • Identify a patient who needs a HMR
  • GP obtains consent and refers HMR to an accredited pharmacist or pharmacy.
  • Pharmacist conducts the patient interview
  • Pharmacist compiles the HMR report
  • GP and patient discuss a subsequent management plan & Item 900 is then claimed

* From October 1 2011 GPs will be able to refer directly to Choice QUM
